Creating a Form
Forms let anyone fill something out and send it to your church, connect cards, prayer requests, volunteer sign-ups, RSVPs, and more. Each form gets its own public link you can share anywhere.
Before you begin
- You need the Admin or Staff role.
Step-by-step
- Go to the Forms page. In your dashboard, open Forms.
- Click New form. Give it a Name, and an optional Description (shown above the form on its public page). The description can be several paragraphs, and your line breaks are kept. Click Create form to open the builder.
- Add your questions. Click Add question, choose a question type, and write the question. Repeat for each thing you want to ask. You can drag questions to reorder them.
- Add text between questions (optional). Click Add text to add a heading, a paragraph, or both, like a section title or a short instruction. There's nothing to answer, and it isn't counted as a question.
- Mark what's required. Turn on the Required toggle for any question people must answer before they can submit.
- Choose who can submit. Under who can fill out the form, pick Anyone, Signed-in users, or Members only.
- Preview it. The phone preview beside the builder shows exactly what people will see. Use the Visitor / Member switch to check both.
- Publish. Click Publish form. Only published forms accept responses, and you'll need at least one question.
- Share the link. Under Public URL, click Copy link to copy your form's short link, like
churchday.com/f/k7m2pq, and share it anywhere, in an email, a text, or on your church page. The short link opens the form directly and never changes, even if you rename the form or edit its address. The longer address shown in Public URL (churchday.com/your-church/f/your-form) works too.
Question types
- Short answer and Long answer: typed text.
- Dropdown, Multiple choice, and Checkboxes: pick from options you set.
- Choice limit (checkboxes): let people pick up to a set number, like "choose up to 3".
- Separate answer (multiple choice and checkboxes): mark an option like None, Not sure, or Not applicable. It shows last, set apart from the others, and on checkboxes it can't be combined with other picks.
- Layout (multiple choice): pick Scale to show the choices side by side in one row, which keeps rating questions like "Not at all true" to "Completely true" short on a phone. Long or many choices show as a list instead.
- Limited slots: options with a set number of spots that fill up and lock when full (great for meal counts or serving spots). See Limited slots sign-ups.
- Yes / No, Number, and Date.
Good to know
- People can change their mind. On an optional one-answer question, tapping the chosen answer again takes it back so they can skip it. A dropdown has a Clear link for the same thing. This doesn't apply to limited slots, where picking another spot is the way to change.
- Text blocks, choice limits, separate answers, and the scale layout also work in event registration and your Join form.
- New people are added automatically. When someone fills out your form and includes an email, they're matched to an existing member or added to your People list, without overwriting anything already there.
- Show a question only when it's relevant. Any question can be set to appear only after someone picks a certain answer to an earlier question ("Show this question only when…").
- Answers can do more than record data. A choice answer can automatically tag people or add them to a group or team, and a question can update the person's profile (like their phone or birthday). Set these up on purpose, check each question's advanced options.
- See your responses two ways. The Submissions tab lists every response with a tally for each choice question. The Analytics tab shows visitors, submissions, and conversion. See Understanding your analytics.
- Deleting a form or a response is permanent and removes all of its submissions.
